Home visit and full assessment, your Aging-in-Place Plan, a 90-day action plan with named owners, the one-page emergency sheet, and a family meeting to walk everyone through it.

For a situation that's stable, but that you'd rather not be watching alone.

Covers monthly check-in, plan maintenance, and family updates. Additional coordination billed at $125/hr if needed.

Comprehensive
$1,050 CAD/month

For a complex or high-stakes situation, or a family carrying this from a distance.

  • Hospital admission and discharge support

Covers full navigation across all care domains. Additional coordination billed at $125/hr if needed.

Not sure which one fits? Most people start with the Roadmap and we recommend a level from there. If none of them fit, we'll say so.

What's not included: LCC does not provide direct caregiving, home support, or personal care services, and we don't make medical decisions on your behalf. We coordinate, advocate, and navigate — working alongside the people already in your family member's life.
Initial term: Monthly plans have a three-month initial term. If you cancel before the initial term ends, fees for the remaining months of that term remain payable. After the initial term, cancel any time with 14 days' written notice.
Travel included: Care package clients are never charged a travel fee — every visit, every zone, no exceptions.

A $99 deposit is required to book The Roadmap or any hourly work. It is applied to your first invoice.

All prices in Canadian dollars plus GST. Hourly work billed in 15-minute increments with an itemized invoice within 7 days of completion. Your first visit is always free of travel charges. For subsequent in-person visits on à la carte or hourly work: Zone 1 (Burnaby and adjacent areas within ~15 km) — no travel fee; Zone 2 (Vancouver, New Westminster, Coquitlam) — $25/visit; Zone 3 (Richmond, North Vancouver, wider Metro Vancouver region) — $45/visit. Government Funding
Many families qualify for support that offsets the cost of care.

BC has income-tested programmes, tax credits, and subsidies most families don't know about. The Roadmap includes a full funding sweep — we identify what you qualify for, and can apply on your behalf.

If something isn't answered here, ask us directly.

Is any of this covered by MSP or extended health insurance?
MSP does not cover care coordination services. Some extended health plans include a care management or case management benefit — it's worth checking your policy. We can provide itemized invoices to support any reimbursement claims.
Do we have to start with The Roadmap?
Yes, if you're joining a monthly plan. The Roadmap is how we get to know your family member's full situation — without it, we can't do the ongoing work well. It's also where the emergency sheet and 90-day plan come from, so most families find it valuable on its own. If you only need à la carte or hourly work, you can book that without a Roadmap first.
What if our situation changes and we need a different level?
You can upgrade to a higher level at any time with 14 days' written notice. Downgrades take effect after your three-month initial term ends. We'll usually be the ones to suggest a change if we think it makes sense — either up or down.
How does billing work?
The Roadmap requires a $99 deposit to book, with the balance due on delivery. Monthly plans are billed in advance on the 1st of each month and have a three-month initial term — we reach out before that term ends to confirm how you'd like to proceed; plans don't continue without your confirmation. If you cancel during the initial term, fees for the remaining months of that term remain payable. After the initial term, cancel any time with 14 days' written notice. Hourly and flat-fee work is invoiced within 7 days of completion. All invoices are sent by email and payable by e-transfer, credit card, or debit.
